(2)SGLT2阻害剤が下記一般式(I)で示される1−チオ−D−グルシトール化合物、その医薬上許容される塩、又はそれらの水和物である、(1)に記載の医薬。

(7)ACE阻害薬が、アラセプリル、ベナゼプリル、カプトプリル、セロナプリル、シラザプリル、デラプリル、エナラプリル、エナラプリラト、フォシノプリル、イミダプリル、リシノプリル、モベルチプリル、モエキシプリル、ペリンドプリル、キナプリル、ラミプリル、スピラプリル、テモカプリルおよびトランドラプリルからなる群から選択される化合物若しくはその製薬学的に許容される塩又はそれらの水和物である、(6)に記載の医薬。
(8)アンジオテンシンII受容体拮抗薬が、ロサルタン、カンデサルタン、イルベサルタン、バルサルタン、テルミサルタンおよびエプロサルタンからなる群から選択される化合物若しくはその製薬学的に許容される塩又はそれらの水和物である、(6)に記載の医薬。
(9)該疾患が高血圧症である、(1)に記載の医薬。
(10)該疾患が糖尿病、糖尿病関連疾患又は糖尿病合併症である、(1)に記載の医薬。
(11)該疾患が、糖尿病及び高血圧を併発している疾患である、(1)〜(10)のいずれかに記載の医薬。
(12)該疾患が糖尿病性腎症である、(1)〜(11)のいずれかに記載の医薬。
(13)該疾患が、さらに腎機能の低下が認められる疾患である、(1)〜(11)のいずれかに記載の医薬。
(14)SGLT2阻害剤と抗高血圧薬とが同時または別々に患者に投与されることを特徴とする、心血管イベントのリスクファクターとして、少なくとも高血圧または糖尿病を含む疾患を治療するための、(1)〜(13)のいずれかに記載の医薬。
(15)SGLT2阻害剤を含み、抗高血圧薬と併用することにより、抗高血圧薬の降圧作用を増強させる医薬。
(16)SGLT2阻害剤を含み、抗高血圧薬と併用することにより、SGLT2阻害剤による糖尿病、糖尿病関連疾患又は糖尿病合併症の治療効果を増強させる医薬。
(17)SGLT2阻害剤を含み、抗高血圧薬と併用することにより、SGLT2阻害剤による糖尿病性腎症の治療効果を増強させる医薬。
(18)心血管イベントのリスクファクターとして、少なくとも高血圧または糖尿病を含む疾患を治療する方法であって、SGLT2阻害剤と抗高血圧薬を同時又は別々にそれを必要とする患者に投与することを含む、前記方法。
(1S)−1,5−アンヒドロ−1−[3−(4−エトキシベンジル)−6−メトキシ−4−メチルフェニル]−1−チオ−D−グルシトール、
(1S)−1,5−アンヒドロ−1−[4−クロロ−3−(4−メチルベンジル)フェニル]−1−チオ−D−グルシトール、
(1S)−1,5−アンヒドロ−1−[4−クロロ−3−(4−メチルチオベンジル)フェニル]−1−チオ−D−グルシトール、又は
(1S)−1,5−アンヒドロ−1−[4−クロロ−3−(4−エチルベンジル)フェニル]−1−チオ−D−グルシトール
であり、さらに好ましくは、(1S)−1,5−アンヒドロ−1−[3−(4−エトキシベンジル)−6−メトキシ−4−メチルフェニル]−1−チオ−D−グルシトールである。(1S)−1,5−アンヒドロ−1−[3−(4−エトキシベンジル)−6−メトキシ−4−メチルフェニル]−1−チオ−D−グルシトールは、好ましくは水和物である。

1錠中に
(1S)−1,5−アンヒドロ−1−[3−(4−エトキシベンジル)−6−メトキシ−4−メチルフェニル]−1−チオ−D−グルシトール(以下、「化合物A」という)2.5mg
リシノプリル水和物 10.9mg(無水物として10mg)
結晶セルロース 79.6mg
D-マンニトール 95mg
カルボキシメチルスターチナトリウム 10mg
ヒドロキシプロピルセルロース 10mg
ステアリン酸マグネシウム 2mg
を含む錠剤を得る。
1錠中に
化合物A 2.5mg
バルサルタン 80mg
結晶セルロース 74.5mg
D-マンニトール 20mg
ヒドロキシプロピルセルロース 5mg
軽質無水ケイ酸 2mg
低置換度ヒドロキシプロピルセルロース 14mg
ステアリン酸マグネシウム 2mg
を含む錠剤を得る。
なお、化合物AはWO2006/073197国際公開公報において実施例7(化合物89)として開示された化合物であり、当該公報に記載の方法により製造することが可能である。
<試験項目>
糖尿病性腎症モデルラットにおける化合物AとACE阻害剤の併用効果
<試験方法>
試験は、2型糖尿病を発症したGoto-Kakizaki(GK)ラットの近交系ラットであり、糖尿病状態が長期に継続した結果、腎臓の組織傷害が進行したラット(T2DNラット)を使用して実施した。1群8〜9例のラット(平均14カ月齢)を用い、化合物A(0.02%、混餌投与)、ACE阻害剤であるリシノプリル(10 mg/kg/日, 飲水投与)を、単独あるいは両薬物を併用して3ヶ月間反復投与した。化合物Aを含まない餌のみを摂取させたラットを病態対照とした。血圧は、投与前及び投与後1ヶ月毎に非観血式自動血圧計を用いてtail cuff法で測定した。反復投与終了後にケタミン及びチオブタバルビタール麻酔下で腎臓を摘出し、10%中性緩衝ホルマリン液に浸漬させて固定した。さらに、腎臓の薄切切片を作製してMason trichrome染色を施した。組織標本をマスク化した後、糸球体を1標本あたり30個抽出した。糸球体病変を定量化するため、以下の基準を設けて傷害の程度をスコア化した。
グレード0: 糸球体に病変が認められない、グレード1: 糸球体毛細血管の1-25%に傷害を認める、グレード2: 糸球体毛細血管の26-50%に傷害を認める、グレード3: 糸球体毛細血管の51-75%に傷害を認める、グレード4: 糸球体毛細血管の76%以上に傷害を認める。
間質繊維化病変は、画像解析装置を用いて1標本あたり10領域の線維化面積率(%)を算出した。
<結果>
(評価項目1:血圧)
結果は、図1に示される。血圧は、収縮期血圧と拡張期血圧から算出した平均血圧((収縮期血圧−拡張期血圧)/3+拡張期血圧))で示した。抗高血圧薬(ACE阻害剤)であるリシノプリルは一定の血圧低下作用を示したが、糖尿病治療薬である化合物Aは単独投与において血圧低下作用をほとんど示さなかった。一方、化合物Aとリシノプリルを併用投与した群では優れた血圧低下作用を示し、この効果はリシノプリル単独投与群と比較しても顕著なものであった。
(評価項目2:腎障害スコア)
糸球体障害スコア
結果は、図2に示される。本実験に用いた動物モデルは、糖尿病状態が長期に継続した結果、腎臓の組織傷害が進行した動物モデルである。この腎傷害動物モデルにおいて化合物Aは、糸球体障害スコアを改善する効果を示した。また、リシノプリルも糸球体障害スコアを改善する効果を示した。一方、化合物Aとリシノプリルを共に投与した群では、各単独投与群と比較してさらに顕著な糸球体障害スコア改善効果を示した。
腎皮質線維化スコア
結果は、図3に示される。化合物Aは腎臓の間質線維化面積を指標にした場合でも腎臓の組織傷害に対して改善効果を示した。一方、リシノプリル単独投与群では間質線維化面積に対する改善効果が認めなかった。それにも関わらず、化合物Aとリシノプリルを併用投与した群では、各単独投与群と比較してさらに顕著な腎皮質線維化スコア改善効果を示した。
<試験項目>
糖尿病モデルラットにおける化合物AとアンジオテンシンII受容体拮抗薬の併用効果
<試験方法>
自然発症の非肥満糖尿病モデル動物であるGoto-Kakizaki(GK)ラット(日本エスエルシー株式会社)に、化合物Aを含有する高蔗糖食を与える。化合物Aの混餌投与期間中に、溶媒またはアンジオテンシンII受容体拮抗薬を単回経口投与して血圧に対する両薬剤の単独効果並びに併用効果をクロスオーバー法にて検討する。収縮期血圧の測定は、アンジオテンシンII受容体拮抗薬の経口投与直前および投与1,2,4,6時間後に無加温型非観血式血圧計を用いてtail cuff法で測定する。
<結果>
結果は、図4に示される。抗高血圧薬(アンジオテンシンII受容体拮抗薬)であるバルサルタンは収縮期血圧に対して一定の低下作用を示したが、糖尿病治療薬である化合物Aは単独投与において血圧低下作用をほとんど示さなかった。一方、化合物Aとバルサルタンを併用投与した群では優れた血圧低下作用を示し、この効果はバルサルタン単独投与群と比較しても顕著なものであった。
Claims (18)
- SGLT2阻害剤と抗高血圧薬との組み合わせを特徴とする、心血管イベントのリスクファクターとして、少なくとも高血圧または糖尿病を含む疾患を治療するための医薬。
- SGLT2阻害剤が下記一般式(I)で示される1−チオ−D−グルシトール化合物、その医薬上許容される塩、又はそれらの水和物である、請求項1に記載の医薬。

RAは、水素原子、C1-6アルキル基、−ORF、又はハロゲン原子を示し、
RBは、水素原子、水酸基、又は−ORFを示し、
RC及びRDは同一又は異なって、水素原子、ハロゲン原子、C1-8アルキル基、又は−ORFを示し、
REは、(i)水素原子、(ii)ハロゲン原子、(iii)水酸基、(iv)ハロゲン原子で置換されてもよいC1-8アルキル基、(v)−ORF、又は(vi)−SRFを示し、
RFは、ハロゲン原子で置換されてもよいC1-6アルキル基を示す。]
。 - 1−チオ−D−グルシトール化合物が、
(1S)−1,5−アンヒドロ−1−[3−(4−エトキシベンジル)−6−メトキシ−4−メチルフェニル]−1−チオ−D−グルシトール、
(1S)−1,5−アンヒドロ−1−[4−クロロ−3−(4−メチルベンジル)フェニル]−1−チオ−D−グルシトール、
(1S)−1,5−アンヒドロ−1−[4−クロロ−3−(4−メチルチオベンジル)フェニル]−1−チオ−D−グルシトール、及び
(1S)−1,5−アンヒドロ−1−[4−クロロ−3−(4−エチルベンジル)フェニル]−1−チオ−D−グルシトールからなる群から選択される化合物である、請求項2に記載の医薬。 - 1−チオ−D−グルシトール化合物が、
(1S)−1,5−アンヒドロ−1−[3−(4−エトキシベンジル)−6−メトキシ−4−メチルフェニル]−1−チオ−D−グルシトールである、請求項2又は3に記載の医薬。 - 抗高血圧薬がレニン−アンジオテンシン−アルドステロン系抑制薬である、請求項1〜4のいずれか1項に記載の医薬。
- レニン−アンジオテンシン−アルドステロン系抑制薬がACE阻害薬またはアンジオテンシンII受容体拮抗薬である、請求項5に記載の医薬。
- ACE阻害薬が、アラセプリル、ベナゼプリル、カプトプリル、セロナプリル、シラザプリル、デラプリル、エナラプリル、エナラプリラト、フォシノプリル、イミダプリル、リシノプリル、モベルチプリル、モエキシプリル、ペリンドプリル、キナプリル、ラミプリル、スピラプリル、テモカプリルおよびトランドラプリルからなる群から選択される化合物若しくはその製薬学的に許容される塩又はそれらの水和物である、請求項6に記載の医薬。
- アンジオテンシンII受容体アンタゴニストが、ロサルタン、カンデサルタン、イルベサルタン、バルサルタン、テルミサルタンおよびエプロサルタンからなる群から選択される化合物若しくはその製薬学的に許容される塩又はそれらの水和物である、請求項6に記載の医薬。
- 該疾患が高血圧症である、請求項1に記載の医薬。
- 該疾患が糖尿病、糖尿病関連疾患又は糖尿病合併症である、請求項1に記載の医薬。
- 該疾患が、糖尿病及び高血圧を併発している疾患である、請求項1〜10のいずれかに記載の医薬。
- 該疾患が糖尿病性腎症である、請求項1〜11のいずれかに記載の医薬。
- 該疾患が、さらに腎機能の低下が認められる疾患である、請求項1〜11のいずれかに記載の医薬。

- SGLT2阻害剤を含み、抗高血圧薬と併用することにより、抗高血圧薬の降圧作用を増強させる医薬。
- SGLT2阻害剤を含み、抗高血圧薬と併用することにより、SGLT2阻害剤による糖尿病、糖尿病関連疾患又は糖尿病合併症の治療効果を増強させる医薬。
- SGLT2阻害剤を含み、抗高血圧薬と併用することにより、SGLT2阻害剤による糖尿病性腎症の治療効果を増強させる医薬。
- 心血管イベントのリスクファクターとして、少なくとも高血圧または糖尿病を含む疾患を治療する方法であって、SGLT2阻害剤と抗高血圧薬を同時又は別々にそれを必要とする患者に投与することを含む、前記方法。
