Abstract
Description
患者の椎骨内(椎体内)及び/又は椎骨間(椎体間)の脊椎手術においては、癒合ケージならびに他のタイプの物体及び/又は装置がしばしば使用される。椎体間装置を用いる場合、1個以上のそのような脊椎体が椎骨間に配置されて、疾病、外傷、一般的劣化又は先天性障害のせいでそのような物体が必要であるところの隣接する椎骨間の癒合を支援し、促進する。椎体内装置を用いる場合、1個以上の脊椎体が椎骨内に配置される。脊椎装置、たとえば癒合ケージなどは、前、後、横又は横後方から脊椎空間に挿入される。
本発明は、隣接する椎骨の間及び椎骨の中に挿入し、維持するための拡張可能な椎体間及び椎体内装置を提供する。特に、本発明は、第一の半径方向輪郭から第二の半径方向輪郭まで拡張する拡張可能及び/又は動的な椎体間及び椎体内装置の様々な実施態様を提供する。
本発明は、傷害、疾病などにより不安定化した脛骨の間又は中の骨成長を支持、安定化及び/又は促進するための椎体間癒合ケージ又は装置、椎体間/椎体内安定化装置など(以下まとめて脊椎装置)として使用することもできる拡張可能及び/又は動的な椎体間(隣接する椎骨の間)装置、椎体内(椎骨の中)装置及び/又は脊椎安定化装置に関する。特に、本発明は、ヒトの脊柱において使用可能である動的な(拡張可能及び/又は拡張・収縮可能な)椎骨間/椎骨内装置の様々な態様を提供する。本発明の様々な態様それぞれを代表するものとして、図1は代表的な動的椎体装置28を示す。椎体28は、脊椎の下寄り部分20だけが示されているヒトの脊椎に埋植又は挿入された状態で示されている。脊椎装置28は、図1の脊椎部20の上下の隣接する椎骨22、24の間に埋植された状態(したがって、椎体間又は椎骨間)で示されている。物体28fとして示される脊椎装置28は、図57及び58の椎骨に埋植された状態(したがって、椎体内)で示されている。椎骨22及び24は、前に向く部分(図1で見て右側の「A」)及び後に向く部分(図1で見て左側の「P」)を有する。
Claims (32)
- 脊椎空間に挿入するためのインプラントであって、
展開部品と、
展開部品上に設けられた複数のセグメントと
を含み、複数のセグメントそれぞれが、展開部品に対して複数のセグメントの軸方向に圧縮されると、第一の直径から第一の直径よりも大きい第二の直径まで拡張可能であるインプラント。 - 複数のセグメントそれぞれが複数の旋回板を含む、請求項1記載のインプラント。
- 複数のセグメントそれぞれが1〜4個の旋回板を含む、請求項1記載のインプラント。
- 複数のセグメントそれぞれが少なくとも1個の旋回板を含む、請求項1記載のインプラント。
- 複数の旋回板が円弧に沿って展開する、請求項2記載のインプラント。
- 複数の旋回板が、骨、骨材料、骨成長材料、薬、治療薬のための受け器を提供する、請求項2記載のインプラント。
- 複数の旋回板が、展開部品上に設けられた展開板を中心に旋回可能に設けられている、請求項2記載のインプラント。
- 旋回板の旋回位置が第一の直径及び第二の直径を画定し、複数の旋回板それぞれが、第一の直径においてはその縦軸が展開部品の縦軸と同軸になるように配置され、第二の直径においてはその縦軸が展開部品の縦軸に対して垂直(又は少なくとも0〜90°の角度)になるように配置される、請求項7記載のインプラント。
- 展開部品がロッドを含み、ロッドが複数のセグメントの第一のセグメントに当接するヘッド及び圧縮部品を受けるためのねじ部を有し、圧縮部品を介して軸方向圧縮が複数のセグメントに加えられる、請求項1記載のインプラント。
- 複数のセグメントの最後のセグメントと圧縮部品との間に位置するエンドキャップをさらに含む、請求項9記載のインプラント。
- 複数のセグメントが放射線透過性材料で構成されている、請求項1記載のインプラント。
- 展開部品及び複数のセグメントが放射線透過性材料で構成されている、請求項1記載のインプラント。
- 複数のセグメントの少なくとも1個が複数のセグメントの他のセグメントに対して半径方向に移動可能である、請求項1記載のインプラント。
- 複数のセグメントが互いに対して半径方向に移動可能である、請求項1記載のインプラント。
- 複数のセグメントが互いに対して半径方向に固定されている、請求項1記載のインプラント。
- 椎骨内/椎骨間埋植のための脊椎体であって、
展開部品と、
展開部品上に担持され、互いに中に軸方向に収められた複数の拡張構造と
を含み、複数の拡張構造が圧縮位置にあるときには、第一の軸方向長さ及び第一の半径方向寸法を画定し、複数の拡張構造が複数の拡張部材の軸方向圧縮のせいで拡張位置にあるときには、第一の軸方向長さよりも小さい第二の軸方向長さ及び第一の半径方向寸法よりも大きい第二の半径方向寸法を画定する脊椎体。 - 複数の拡張構造それぞれが複数の旋回板を含み、各旋回板が圧縮位置及び拡張位置に旋回可能である、請求項16記載の脊椎体。
- 複数の旋回板が円弧に沿って展開する、請求項17記載の脊椎体。
- 複数の旋回板が、展開部品上に設けられた展開板を中心に旋回可能に設けられている、請求項17記載の脊椎体。
- 複数の拡張構造それぞれが1〜4個の旋回板を含む、請求項16記載の脊椎体。
- 旋回板が、骨、骨材料、骨成長材料、薬、治療薬のための受け器を提供する、請求項20記載の脊椎体。
- 旋回板が拡張位置でスナップロックする、請求項17記載の脊椎体。
- 展開部品がロッドを含み、ロッドが複数の拡張構造の第一のセグメントに当接するヘッド及び圧縮部品を受けるためのねじ部を有し、圧縮部品を介して拡張構造に加えられる軸方向圧縮が拡張構造の圧縮位置を拡張構造の拡張位置に変える、請求項16記載の脊椎体装置。
- 複数の構造の最後の構造と圧縮部品との間に位置するエンドキャップをさらに含む、請求項23記載の脊椎体。
- 複数の構造が放射線透過性材料で構成されている、請求項16記載の脊椎体。
- 複数の構造の少なくとも1個が複数の構造の他の構造に対して半径方向に移動可能である、請求項16記載の脊椎体。
- 複数の構造が互いに対して半径方向に移動可能である、請求項16記載の脊椎体。

- 脊椎体を椎骨間又は椎骨内空間に導入する方法であって、
展開部品及び展開部品上に設けられた複数のセグメントを含み、複数のセグメントそれぞれが、展開部品に対して複数のセグメントの軸方向に圧縮されると、第一の直径から第一の直径よりも大きい第二の直径まで拡張可能であり、はじめに第一の直径を有するインプラントを用意するステップと、
第一の直径のインプラントをカニューレに挿入するステップと、
カニューレの開口を椎骨空間に隣接して配置するステップと、
第一の直径のインプラントを延ばしてカニューレから出し、椎骨空間に入れるステップと、
インプラントに軸方向圧縮を提供して、インプラントを第二の直径にするステップと
を含む方法。 - 複数のセグメントそれぞれが、セグメントの展開板を中心に旋回可能に設けられた複数の旋回板を含み、展開板が展開部品上に設けられている、請求項29記載の方法。
- 旋回板の旋回位置が第一の直径及び第二の直径を画定し、第一の直径においては、複数の旋回板それぞれがその縦軸が展開部品の縦軸と同軸になるように配置され、第二の直径においては、複数の旋回板それぞれがその縦軸が展開部品の縦軸に対して垂直になるように配置される、請求項29記載の方法。
- 展開部品がロッドを含み、ロッドが複数のセグメントの第一のセグメントに当接するヘッド及び圧縮部品を受けるためのねじ部を有し、圧縮部品を介して軸方向圧縮が複数のセグメントに加えられる、請求項29記載の方法。
