Unusual behavior on code action, show different options on each time. when click Add missing return types it does not do anything and sometimes adds namespace only on class.

I'd probably need to see the Phpactor Language Server Output log. Set "phpactor.trace.server": { "verbosity": "verbose" }, in your projects settings.json and reload vscode
