: Collection of the data in the Relay study was approved by the Danish Data Protection Agency (The Region of Southern Denmark project-ID 2008-58-0035). Formal informed consent was not required from the Regional Scientific Ethical Committees of Southern Denmark as the study was considered a register study that did not entail an intervention (Project ID: S-20130084). Collection of data from the Electronic Health Records was approved by the Danish Patient Safety Authority (Project-ID 3-3013-1601/1) and the Danish Data Protection Agency (The Region of Southern Denmark Project-ID 16/12126). All information was anonymized to ensure confidentiality and that all methods were carried out in accordance with relevant guidelines and regulations.
