HealthChoice Help for Members

​Our HealthChoice Helpline phones are temporarily unavailable. Use the online form for help with a HealthChoice managed care health plan, provider, or service.

You will get an email confirming we have received your form. An agent will respond to you within 24 to 48 hours, Monday through Friday between 8:00 a.m. and 5:00 p.m. We will reply to you on a holiday or weekend within the next three business days.

Do not use this form for a medical emergency. For emergency care, go to your nearest hospital's emergency room (ER). If you think the problem is life-threatening, call 911.
